Translates an OutboundRequest into an OutboundAddress + Msg and pushes
it through the ChannelManager.
Used by both OutboundController (HTTP) and OutboundTool (agent tool). Errors
are surfaced as IllegalArgumentException (bad request) or IllegalStateException
(no such channel / channel unhealthy).
